New MVR and EDC Forms

EXCITING NEWS!

We are excited to announce the release of two new online Microsoft forms that replace the long-standing PDF forms:

These new versions eliminate the need for the previous PDF forms, as well as help streamline and digitize the MVR Request and EDC, which is RMI’s initial step in improving this process. The forms are user-friendly and self-explanatory to complete and will automatically route according to the information entered in the forms. Please be sure to update your internal procedures and provide the new online form links to your MSU vehicle drivers.  

We want to take this opportunity to provide additional information and clarification regarding the Employee Driver Certification (EDC) and the Motor Vehicle Record (MVR) Review Request, and the differences and purposes of each:

Employee Driver Certification (EDC) – The EDC is required annually by employees/student employees who are assigned to drive MSU vehicles. Departments are responsible for reviewing and maintaining these submitted forms in their unit, and the EDC is not to be used for ordering MVR reviews from RMI. If the employee falls into a category where an MVR review is required, or if a department specifically wants their record reviewed, they would still submit the EDC form, in addition to the MVR Review Request form, as the forms serve two separate purposes.

Motor Vehicle Record (MVR) Review Request – The new MVR Review Request is be submitted by the driver to RMI for processing. 

  • Faculty/Staff: Recommended that all faculty/staff authorized to drive an MSU vehicle have their MVR reviewed annually by RMI.
  • Students: Required that all students have their MVR reviewed and approved by RMI at least once per year to operate an MSU vehicle. MSU vehicles may not be used by students unless they are approved by a University department.
